REQUIRED FORMS
Once your child has been offered a spot and the registration fee has been paid, you will be contacted to pick up the enrollment packet and required forms, or you may choose to have them mailed to you. All forms must be completed and returned before your child can begin attending.
Within 30 days of enrollment, each child must have a current medical report and an up‑to‑date immunization history on file in the MBCC office. These documents are required by state regulations and must remain current throughout the year.
